if you can receive a call, i doubt the problem is anywhere in your CMTS, provisioning (dhcp tftp etc..) and lies in one of two possible places.
1. your digitmap is incorrect so that it is not passing the required digits to your carrier, i.e. your area requires 10 digit dialing and your only passing 7.....
2. whatever your MTA is talking to metaswtch, asterisk etc.. is not configured to route calls to the outside properly.
the only one of which you actually control being the Digitmap, it usually is a snmp variable in the mta config.
