quick check, see that the upstream modulation is qpsk with 3.2Mhz wide. the most you would get is about 3.5Mbps because of docsis overhead. I recommend you try modulation profile 2 (16Qam) instead of 1 (qpsk).
Also the modem file, upstream concatenated burst should be 10,000 and max burst 20,000
