I am trying to find solution for my issue by reading troubleshooting manuals, config guides, but not successfully for now.
I have one issue related to the cmts Casa C3200 and no other support than google, etc... ;)
Here is my problem:
We have C3200 with all 6 modules...3x QAM and 3x UPS and the issue is related to module 5.

If I plug QAM card in slot 5 it doesn't boot successfully.
Here is the log:
[Tue Jan 10 12:20:03 2017]-ER-SYS-1: Slot 5 failed to become startup after 3 tries
[Tue Jan 10 12:17:01 2017]-AL-SYS-1: No available redundant card for slot 5
If I swap UPS from module 3 to module 5, it also doesn't boot. The LED status from the front side in "amber" which mean Booting, or otherwise functional, but not ready.
In config guide is written that module 5 and 8 are for HA redundancy...but I need all six slots(0-5) for use in the same moment.
Do you have any experiance, solutions for my issue?

5 and 8 for redundancy is for the C10G and C100G chassis, here is the troubleshooting procedure for the c3200 for the problem listed from the hardware install manual
Line card in "Not ready" state
If a CMTS line card is in the “Not ready”state, it is most likely a hardware problem. However, a failed process in the operating software will also result in the “Not Ready” state. Casa Technical Support can determine if this is the actual problem.
If the line card is in the "Not ready" state, first ensure that the line card is properly seated in the slot and that the screws are tight. Otherwise, a failure may be present at the line card, card cage, or at the SMM. The first step should be to try to reboot the line card unconditionally from diagnostic mode, as follows: (diag)#
reboot module x unconditional
If these actions do not resolve the problem, swap the line card. If the problem persists, swap the card cage. If this still does not resolve the problem, then swap the SMM.See Chapter 4, “Maintaining the C3200 hardware” for information on replacing the SMM and the card cage, if necessary

I also swap line card UPS from module 1 to module 5 and in module 5 that UPS card also doesn't work, but QAM in module 1 works.
The status for module 5 is the same. So I suspect that the problem isn't related to line card, but to the card cage, smm or software?

Is it ok, that SW on C3200? I don't know why it says that modules 5 and 8 are for redundancy if I have HW C3200. Is it possible that I have SW for C10G on my HW?
Could someone please send me the newest SW than I have?
