First we are not able to provision IP DHCP on MTA. The provision server provide to CM MTA an IP address but the modem seems not to accept it.
Anyone have an SLA or documentation to provision VoIP on this modems ?

With MTAs not accepting DHCP, it almost always is because the dhcp response did not contain all of the options the MTA requested. Use wireshark to check what options the MTA is requesting (option 55) and make sure your response has all of those options. The options that usually trip people up are option 12 and 15 the hostname and domain name. Also on some MTAs the provisioning servers dns name must be resolvable.
