My Motorola SBG940 Stuck on init(i).

After ACK modem do not send request to the ToD or TFTP servers. Log from Moto web shows:
3-Critical R004.00 Received Response to Broadcast Maintenance Request, But no Unicast Maintenance opportunities received - T4 timeout
3-Critical D005.00 TFTP failed - request sent - No Response
4-Error D004.03 ToD request sent- No Response received
3-Critical R002.00 No Ranging Response received - T3 time-out
3-Critical T001.00 SYNC Timing Synchronization failure - Failed to acquire QAM/QPSK symbol timing
CMTS IOS : ubr7200-k9pu2-mz.123-23.BC7
Moto SW_REV: SBG940-2.1.15.0-SCM00-NOSH
Uses CORE and isc-dhcp servers.
Any idea?

A few remarks, not sure if it help, but: add "cable tftp-enforce" in Cable 3/0; change time-offset from "0" to say "300" just to be on a safe side :) . Can You ping the modem from cmts & dhcp server? Did You set full fqdn provisioning? Have You got smth like this:ip dhcp relay information option;
no ip dhcp relay information check in cmts config?

Hard to say exactly but could be a few things:
1. Tftp server is it running
post the output of
netstat -panu | less
do the files exist in the directory, if your using the built-in linux tftp server run
ls -al /tftpboot so that we can check your permissions.
Your dhcp Offer actually points to two different files, moto.cm and gold.cm, some firmware versions want the old bootp filename and server names ( filename "moto.cm"; and next-server 192.168.0.6;) and most want the dhcp version (option bootfile-name "gold.cm"; , and option tftp-server-name "192.168.0.6";) which is not normally an issue you can give both pieces of info just make sure they match.
2. Your tftp server cannot communicate with your modem ips. When the modem is trying to come online try pinging it from the tftp server if you cannot ping it then tftp will not work and you will need to troubleshoot the connectivity between the two.
your likely culprit for init(i) is one of these two quite likely a tftp issue.

After I set in TFTP server on uBr, sbc940 get the file from cmts and going to online but the main problem still exist.
I have access from cmts, can ping or snmp, but have no access from devices connected after the router, eg provisioning or gateway computers, so it looks like a roting problem. All other modems ( D1.0...3.0) works fine, both with internal and external tftp/ToD ...
Actually The same issue have with other routing CMTS's

Have Tou tried this trick? :

ip route 10.10.xxx.xxx 255.255.0.0 GigabitEthernet0/2
on UBR of course, where GE0/2 is connected with provisioning.
