This DOCSIS analyzer is a really a 'ruggedized' PC with a special DOCSIS card. I wouldn't call it an embedded cable modem because it runs in a 'promiscuous mode' and so can look at all the up and downstream traffic, as well has some special features related to capturing burst traffic.
I did end up finding two sources, one of them being TVC which has a Sigtek 261-B. The other one you'll have to ask me about.

The form factor on the ST-261 is nothing special: just a 'ruggedized' PC, monitor, and keyboard in a largish box:
