Apex1000 ***FAILURE - Error programming MUX FPGA(s)

I started getting this error and the QAMs are not seen anymore. I just recently swapped out the QAMs with known good ones from ones that were failing and everything looked fine with the known good ones until I rebooted the unit and now I am seeing the following on bootup.

Do you think this is a hardware issue, or could it be possible that the config has become corrupted some how?
